If you are a carer, it is important that the practice is informed. It will allow us to offer you the right information, support and access to services.
If you need to register as a Carer, you can download the attached form and either email it back to us at the surgery once it is completed and signed or hand it in to the reception team. Alternatively, you can collect a form from the reception team and drop it back off to us when it’s completed.
Swansea Carers Centre is a local charity which gives much needed support and information for carers across Swansea by providing:
- welfare benefits advice
- respite care
- access to special grants and funds
- advocacy services
- counselling services
- carers mutual support groups
- training
