You do not need a ‘fit note’ from a medical professional if you are off work, because of injury or illness, for 7 days or less. In this instance, you may complete a ‘self-certification’ form. These forms are available from the reception team, or they may be available from your employer.

If you are absent from work for longer than 7 days, you will need to contact the surgery to request a medical certificate or ‘fit note’. There is no charge for fit notes if you have been off work for more than 7 days. The doctor may charge a fee if a fit note is requested earlier than 7 days.

If you are self-isolating and cannot work because of coronavirus (COVID-19) you can get an ‘isolation note’ online from NHS 111 (https://111.nhs.uk/isolation-note/). You will not need to request a ‘fit note’ from the surgery.
